New Patient Registration
If you wish to register with the Practice you can check to see if your address is within the Practice catchment area here: Practice Area Map
To register with the Practice you will need to complete the online registration form or alternatively print and complete the GP Registration Form (PDF, 313KB).
You will also need to complete the Patient Registration Pack (DOCX, 232KB) which includes an application for online access.
Forms are also available from Reception.
Once you have completed the registration form please provide the Practice with two forms of identification; something that confirms your address, eg utility bill or council tax bill, and a driving licence, passport or birth certificate. If you have an NHS medical card please bring this with you also.
Once you have been accepted as a patient your medical records will be transferred to the Practice.
Named Accountable GP
On registering with the Practice you will be allocated a named accountable GP who is responsible for your overall care at the practice. Patients are currently allocated to whichever GP has the shorter list of patients. Please ask reception if you wish to know your named GP. Please note this does not affect your ability to see any of the doctors working within the practice. If you have a preference as to which GP will be your named accountable GP please inform reception and we will make all reasonable efforts to accommodate this request.

Temporary visitors and tourists
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
Homeless patients
Homeless patients are entitled to register with a GP using a temporary address, which may be a friend’s address or a day centre. The practice address may also be used to register them.
The practice has a non-discrimination policy for accepting new patients. There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case, simply choose another GP in your local area.
If English is not your first language, the practice has access to interpreters. Please inform reception staff and this assistance can be provided.
