Registering as a Patient at Much Wenlock and Cressage Medical Practice

New Patient Registration Forms can be collected from the Reception Desk at Much Wenlock and Cressage Practices or downloaded below:

Alcohol Audit Form for completion - new patients

Please return the completed forms to the Practice Reception Desk, along with 2 forms of Identification.

PLEASE NOTE: We are unable to process Registration Forms sent via email

Please also provide at least two forms of ID (one photographic and one current utility bill). 

If you have your NHS medical card, please bring this with you also.

Once we have received your registration documentation one of our receptionists will enter them onto our computer system.

On registering with the Practice you will be allocated a named accountable GP who is responsible for your overall care at the practice.  Patients are currently allocated to whichever GP has the shorter list of patients.  Please ask reception if you wish to know your named GP.  Please note this does not affect your ability to see any of the doctors working within the practice. 

If you have a preference as to which GP will be your named accountable GP please inform reception and we will make all reasonable efforts to accommodate this request.

By completing the registration process you are registered with the practice and your previous records will be requested and processed by us as quickly as possible.

Registering as a Temporary Patient

You can register as Temporary Resident in order to recieve medical attention if you are away from your perminant residence.
